Undergraduate Scholarship Application


APPLICANTS MUST BE MEMBERS OF PHI UPSILON OMICRON AND ENROLLED IN A COLLEGE/UNIVERSITY BACCALAUREATE DEGREE PROGRAM

 Available Scholarships:  Carefully review the criteria for each scholarship and apply for all of those for which you are eligible. Scholarship recipients must be enrolled as fulltime students for the semester(s) in which scholarship money is received.

The following Scholarship is available to Phi Upsilon Omicron members to be enrolled during the full 2006-2007 academic year:  

Margaret Jerome Sampson Scholarships  - preference given to dietetics and/or food and nutrition majors.  Must provide a financial statement and one double-spaced typewritten page explaining the need for additional financial assistance. 

The following Scholarships are available to Phi Upsilon Omicron members to be enrolled for the fall 2006 academic quarter/semester OR during the full 2006-2007 academic year:

Jackman Scholarship - preference given to majors in family and consumer sciences education who have shown exemplary commitment to Phi Upsilon Omicron

Lillian P. Schoephoerster Scholarship - must be non-traditional student.  Must provide a financial statement and one double-spaced typewritten page explaining the need for additional education for gainful employment and giving reasons why to be considered a viable candidate.

Martha Combs Jenkins Scholarship - preference given to student who has shown exemplary commitment to Phi Upsilon Omicron 

Mary Weiking Franken Scholarship -preference given to majors in child and/or family and consumer sciences education
Nell Bryant Robinson Scholarship - preference given to majors in dietetics and/or food and nutrition
Past Presidents Scholarship

S. Penny Chappell - preference given to majors in fashion design and construction, textile design and development, and/or textile preservation
Sutherland/Purdy Scholarship - preference given to majors in clothing and textiles or a related area such as apparel design or fashion merchandising
Tommie J. Hamner Scholarship - preference given to student who has shown exemplary commitment to Phi Upsilon
Omicron
Treva C. Kintner Scholarships - must be non-traditional student who must have completed at least one half of credits required  for degree  

General Directions

1. Application package must be postmarked on or before February 1, 2006.
2. All requested documents must accompany the application, not be sent separately.
3. All application documents must be typed on regular typing paper, not handwritten.
4. Submit one (1) original of all information requested.  A separate application must be submitted for awards.
Please collate.  Applicants may apply for more than one scholarship.  Only one application is necessary even for more than one scholarship.  Be sure to list the name(s) of all scholarships for which you apply.
5. All application documents are to be assembled in the order described below, clipped together with a paper clip, placed in a standard size manila file folder with tab, and then placed in a 10"x13" envelope (regular or priority envelope) for mailing.

Application Directions  

1. Complete and submit application information in the following order:  application, transcript(s), recommendations (in order as stated on application). Use the correct format for each  section.
2. Submit one official transcript from each college/university attended (if latest transcript does not include previous information). To be official, the Registrar of the University is    requested to place the transcript in an envelope, seal it, write his/her signature across the seal, and give (or mail) to student for inclusion in application folder.
3. Request three recommendations. List the names of the three persons whose references you are including.  Include a current or previous academic advisor and a current or previous professor.  To be official and confidential, each person writing a letter of recommendation should be requested to place the letter in an envelope, seal it, write his/her signature across the seal, and give to student for inclusion in the application folder.
4. Enclose a self-addressed stamped postcard.  It will be returned when the application is received.
5. On or before February 1, mail completed application package to Phi Upsilon Omicron, Inc.  P.O. Box 329,   Fairmont, WV  26555.

Recommendations

List the names of the three persons whose references you are including.  Include a current or previous academic advisor and a current or previous professor. (NOTE: Please refer to item 3 under Application Directions to ensure that the letters of recommendation submitted are official and confidential.) Recommendations need to address character, leadership, service and ability.  Financial need should be addressed in recommendations for Sampson and Schoephoerster scholarships.

  Rules Governing Phi Upsilon Omicron undergraduate scholarships
1. Recipients will be selected without bias toward race, creed, gender, national origin or age.
2. Alternate recipients will be selected and notified if the original recipient is unable to accept.
3. Picture and resume will be requested of undergraduate scholarship/award recipients for publicity purposes.
4. Scholarship payments will be sent to the unit administrator.  Recipients must be enrolled as fulltime students for the semester(s) in which scholarship money is received.
