Undergraduate Orinne Johnson Writing Award Application

 

APPLICANTS MUST BE MEMBERS OF PHI UPSILON OMICRON 

GRADUATING SENIORS ARE ELIGIBLE

 

Orinne Johnson Writing Award - applicant must submit creative writing related to the current professional project theme for publication in THE CANDLE.

General Directions

1.  Application package must be postmarked on or before February 1.

2.  All requested documents must accompany the application, not be sent separately.

3.  All application documents must be typed on regular typing paper, not handwritten.

4.  Submit one (1) original of all information requested.  Please collate.  A separate application must be submitted for scholarships and other awards.

5. All application documents are to be assembled in the order described below, clipped together with a paper clip, placed in a standard size manila file folder (with tab), and placed in a 10"x13" envelope (regular or priority envelope) for mailing.

 

Application Directions

1.  Complete and submit application information in the following order: application information, creative writing

2.  Enclose a self-addressed stamped postcard.  It will be returned when the application is received.

3.  On or before February 1, mail completed application package to Phi Upsilon Omicron, Inc., P.O. Box 329,      Fairmont, WV 26555.

 

Application Information

Prepare a cover page that is to include the following information:  (Please type each heading and subheading and then requested information.)

1. List the name of award (Orinne Johnson Writing Award)

2.  Personal Information

Name (last, first, middle or maiden)

               Address (including street, house or apartment number, city or town, state, ZIP) phone, fax, e-mail (current)

Address, phone, fax, e-mail (permanent)

Phi Upsilon Omicron Chapter (where initiated, then month and year)

Membership Verification (As verification, include your Chapter Advisor’s signature)

3.  Classification

Major

Year and Term (e.g. junior, second semester)

Degree Currently Pursued

Expected Graduation Date

4.  Academic Information

Institution (college, university) where degree is being, or will be pursued.

Overall Family and Consumer Science Unit name (e.g. Human Ecology) not subunit.

Address, phone, fax, e-mail of overall unit.

Administrator of overall unit (name, position, title e.g. Dean, Ph.D.]

5. Applicant Responsibility

Important:   Applicants are responsible for notifying the Phi Upsilon Omicron National Office if status should change from the information provided on the application (e.g. name, address, institution).

Please indicate in a statement, which includes your signature, your understanding and willingness to comply with the request.

                                                           

Creative Writing

1. Creative Writing

Applicant must submit creative writing related to the current professional project theme for publication in THE CANDLE. Submission should be 750-1000 words, doubled spaced. 

2. Documentation

Quotations must be properly documented using APA format. 

3. Criteria

Writing will be judged on (1) relationship to current professional project theme, (2) organization and cohesiveness, and (3) grammar, spelling and punctuation.

Rules Governing Phi Upsilon Omicron Awards

1.  Recipients will be selected without bias toward race, creed, gender, national origin or age.

2.  Alternate recipients will be selected and notified if the original recipient is unable to accept.

3.  Picture and resume will be requested of award recipients for publicity purposes.
